I am a published mathematician nearing the completion of their PhD who has structured their education and work experiences around success as a future math professor. I have taught a wide variety of courses ranging from high school to master's level to individuals enrolled at my local college while finishing my PhD.

I conduct a brief diagnostic session first to help me assess the expectations and needs to the student, at which point a tutoring plan with be discussed. If both parties agree to the plan, the first session can begin at that time.
College age students and Adults from a wide variety of backgrounds and skill levels. The majority of my students are college freshman.
There are a wide variety of teaching and learning styles. Meet with them first to address that, and don't be afraid to look elsewhere if the fit isn't there. Any good instructor with your best interests at heart should have no problem with you seeking help elsewhere if you don't feel like the styles match up.

Details
The diagnostic meeting is $10, however that cost is taken off of the next session if the student decides to take me on as an instructor or if I decide that it is in the student's best interests to seek help elsewhere.
Otherwise my pricing is tiered based off the number of hours a week we meet. $40 for one hour, $70 for two, $90 for 3. It is unlikely but not impossible for to suggest more than three hours a week of meeting, but if that is what the student wants pricing will be discussed privately.
